This document helps you to keep track of what you have completed and returned to the office for your employment for the summer. (If your information has not changed from last year, you do not need to return a W-4, I-9, or Berkheimer Residency Forms.
>>Employee Records Memorandum
You acknowledge that there are no changes to your W-4 and that there are no unemployment funds taken from your salary.
>>Health Form
To ensure your health to fulfill the duties of your summer staff role.

This is necessary if you have a health issue develop while at camp.

So Twin Pines knows who you parents/guardians will permit you to travel off camp property.

This is a program where you attain supporters of your ministry work for the summer.

KITCHEN STAFF PACKING LIST

The registration process for Lotsa Love involves two forms. The Pre-Registration form helps us determine whether or not we are able to meet the camper’s needs and if the camper can handle the program. To participate in this experience, campers must be able to communicate their basic needs, be toilet-trained, and be ambulatory. Once the camper is approved, the regular registration form can be completed.
